Group Change Form: If you need to update the information for your group with the District, Area, and World Service Office, use this form. You'll want to use a group change form if

• Your group elects a new group representative
• You form a new group
• Your group changes the time or place of it's meeting
• You disband your group

Don't forget to send a copy to your District Representative (DR), Area Group Records Coordinator, and the World Service Office. If you live in a district that is serviced by an Al-Anon Service Center (ASC) then you should also send them a copy. Addresses include:
